I am an out-of-network provider. Depending on your plan, services may be covered in full or in part through out-of-network benefits or an Employee Assistance Program (EAP).

 

You may wish to ask your insurance representative:

• Do I have mental health benefits?

• What are my out-of-network benefits?

• Is pre-authorization or pre-certification required?

• What is my deductible, and has it been met?

• How many sessions are covered per year?

• What percentage of each session is reimbursed?

• Is a referral required from my primary care physician?

 

Upon request I will provide you receipts to submit to Insurance for reimbursement.

 

Payment

Payment is due at the time of each session. Accepted forms of payment include credit card (processed through HIPAA-compliant Ivy Pay), cash, and personal check.

 

Cancellation Policy

24 hours’ notice is required for cancellations to avoid being charged for the scheduled session.
